FEES AND INSURANCE

I am an out-of-network provider for insurance plans.  All PPO and some HMO insurance providers have out-of-network benefits. Please contact your insurance provider directly to verify your out-of-network benefits and how you may be reimbursed for some of the costs associated with my services. I am happy to submit your claim to your insurance company electronically or I can provide you with a monthly statement for reimbursement for services rendered. Payment is due at the time of service.
